Please note: We are working with additional Durable Medical Equipment (DME) vendors, Byram Healthcare and Apria Healthcare to provide more in network DME options. In addition, we have a national contract with Labcorp and Quest, which may not be reflected in our printed directory. We are working to obtain facility information for our directories at this time.

Network Coverage Rules
You can choose to receive care from out-of-network providers. Our plan will cover services from either in-network or out-of-network providers, as long as the services are covered benefits and medically necessary. However, if you use an out-of-network provider, your share of the costs for your covered services may be higher.
